What we collect when you visit, fill out a form, or give — and what happens to it after.
Effective October 13, 2026
Sylinda M. Wilson operates this website on behalf of Perfectly Unperfect Minds®. If you have questions about your information, email perfectlyunperfectminds@gmail.com or call (470) 804-1047.
Perfectly Unperfect Minds® is a youth mental health education and advocacy initiative. We are not a clinical treatment provider. We do not provide therapy, clinical assessments, diagnoses, or emergency mental health services.
We ask only for what we need to respond to you. Depending on how you use the site:
We do not run advertising trackers or ad scripts on this site, and we do not sell your information.
Our Refer a Youth form is a Google Form. Answers are processed and stored by Google in our organization's Google account, not on this website. The form asks for:
Please share only what's needed. Do not include medical records, diagnoses, or other detailed health information.
A referral is not enrollment and is not consent to participate. Submitting a referral lets us contact the appropriate adult about available programs. It does not guarantee a place in a program, and a minor does not take part in programming without parent or guardian permission, unless an applicable legal exception allows otherwise.
We serve adolescents ages 12–18. Young people may tell us they're interested in our programs, and we try to collect as little identifying information from minors as we can.
Parents and guardians can reach us at any time at perfectlyunperfectminds@gmail.com or (470) 804-1047.
This policy explains how we handle information. It does not replace parent or guardian consent or our safeguarding procedures.
Only Perfectly Unperfect Minds® team members who need it. Website form submissions can be viewed only in a sign-in-protected admin area. Google Form responses can be viewed only by people with access to our organization's Google account. We don't sell, rent, or trade your information.
These companies process information for us, and their own privacy policies also apply:
We keep information only as long as we need it to respond, follow up, and meet legal or accounting obligations, such as donation records. We are finalizing a written retention schedule and will update this section when it's in place. You can ask us to delete your information at any time (see below).
This website uses a secure (HTTPS) connection. The public can submit website forms but cannot read submissions, and admin areas require a sign-in. No method of storing or sending information is completely secure, so please share only what's needed.
A parent or guardian, or anyone who submitted a form, can ask to see, correct, or delete the information we hold. Email perfectlyunperfectminds@gmail.com or call (470) 804-1047 with the name used on the form and what you'd like us to do.
We'll ask a few questions to confirm your identity — and, for a young person's information, that you are their parent or guardian — before we share or change anything. Some information may need to be kept where the law requires it, such as financial records.
You can also stop receiving email from us by replying and asking to be removed.
We cannot promise absolute confidentiality. We may share information without consent when the law requires it, or when we believe it's necessary to respond to an immediate concern about someone's safety — for example, by contacting emergency services, a parent or guardian, or child protective services.
We link to resources we didn't build — crisis lines, Google Forms, our social media pages. Their privacy practices are their own, so it's worth reading theirs.
We'll post the new version here and update the "Effective" date at the top. Material changes will be announced on the site.
This site is not a crisis service. If you are in danger or thinking about harming yourself, call or text 988, text "GA" to 741-741, or call 911. Those services are free, confidential, and available around the clock.
